Urologist or oncolog: for localized prostate cancer, a urologist is the first stop (although he may recommend radiation therapy. For metastatic prostate cancer, a medical oncologist is first stop. You don't mention where the patient lives. When prostate cancer is not cured, it requires ongoing care, best given by a doctor close to home.

When the cancer has spread you need a team approach with a urologist and medical oncologist; I advise using a urologist that specializes in urologic cancers: urologists specialize in areas like cancer, pediatrics, female urology, and reconstruction.
