Vagal : Likely a vagally-mediated etiology. The mechanism of of hypotension could come from afferent impulses from the upper gastrointestinal tract and efferent impulses to the heart that can produce a variety of bradyarrhythmias with atrioventricular block. This would result in low BP and symptoms mentioned.
