Statin drug therapy: Hi. When you say "high Cholesterol" we're assuming you mean high LDL (and maybe high non-HDL, given your fatty liver) cholesterol. High LDL responds VERY poorly to diet and exercise. With a high LDL and a positive FH of CAD, I recommend you take a statin drug. There is a wealth of data for the use of statins in that situation. If you're diabetic, treat that aggressively. Same for BP. Don't smoke!

You'll need Rx: Despite all the grandstanding about "healthy lifestyle", diet and exercise will fail if you have a strong genetic tendency to hypercholesterolemia. This isn't your fault. It's time to ask your physician about a statin or other medication. Ignore the anti-statin "natural" websites, keep up the physical fitness and healthy diet stuff for its other benefits, and accept what you need to do.
