Prior authorization: A patient, who has been recommended to undergo a certain procedure may ask her doctor to obtain "prior authorization" from her insurance carrier. The doctor will need to provide a diagnosis code and a procedure code and the insurance carrier will then determine, whether the procedure is covered under your plan.
