Tough problem: Approach here is to try a number of different things. Sometimes a lumbar corset can help. Topical pain patches that have Lidocaine can help. Sometimes xray guided pain injections can help. Finally low doses of narcotic medication as a last resort but can have significant side effects in someone 85.

Varies.: If simple oral medications and the use of a corset brace can control it, than that is fine. If not we often use our colleagues in interventional pain management for their help. They often will try some form of injection therapy. I often will get an MRI just to see if there might be a minimally invasive approach to their problem, such as in an 85 year old with scoliosis but also a herniated disc.
