Worse = return to ER: Something may now be present that was not at first visit, or even if not, we know you are not a skilled nursing facility and some behaviors exceed the ability to adapt to overnight at home. If worsening and concerned, best answer is to return to ER (preferably same one so have labs to compare against and do not have to completely repeat workup).

Back to ER: Go back, he's worse! If no good answer given and he;s no better, request transfer to another hospital. How scary for you. Good thing he has you! Get going. Be calm, polite and firm about getting the answers you need to go forward.
