Surgery: When it's visually significant, and vision can be improved, then cataract surgery is recommended. Otherwise avoid uv exposure, don't smoke, etc.

Surgery or specs: Vision loss from cataracts can often be corrected by glasses if the cataracts cause refractive changes only. If there is cloudiness from the cataracts surgery is a good option.

Cataract surgery: When vision or glare test (light shined in eye as read chart) is worse than 20/40, cataract surgery is recommended. Always: protect eyes from sun/ uv exposure, don't smoke, eat green leafy veggies.
