The main active chemical in them was called ephedrine, (ephedrine sulfate) and is closely related in structure to our own neurotransmitters epinephrine and norepinephrine (aka adrenaline and noradrenaline), as well as amphetamine and its cousins. It is a decongestant mainly, but also a stimulant, increasing wakefulness, pulse and BP.

Decongestant: Short term ( a couple of days) use as a decongestant is ok. But it does increase blood pressure, so if you have underlying high blood pressure , i would avoid it. And for that reason, shouldn't take it long term, even if you are healthy.

A stimulant: Ephedrine (ephedrine sulfate) is similar to the stimulants the body makes itself - adrenaline, noradrenaline and dopamine, and does many of the same things - raises blood pressure and pulse, is a decongestant, appetite suppressant. Because of its risks, most doctors no longer prescribe it, and use pseudoEphedrine (ephedrine sulfate) (sudafed) instead as a decongestant mainly. It is still found in many herbal drugs, esp from china.
