Conservative then: Surgical.Rest and take something for the pain, ice the sesamoid bones, choose flat shoes, wear an orthotic insole if those measures did not help you may need surgery.

I'd start with...: A "dancer's pad." i don't know why they are called this, but it is a pad worn on the bottom of the foot to accommodate the sesamoids under the first metatarsal head to keep them from bearing as much weight as they usually do. Then otc anti-inflammatory meds such as aleve (naproxen). If these don't help as much as you need, see a podiatrist.
