Need MRI: Sesamoid pain can be caused by avn or atress fractures of the sesamoid. Best test to determine the cause of sasamoid pain unresponsive to orthotics is a mri. Severe cases of sesamoiditis may require wearing a short leg walking boot with pressure relief under the the 1st metatarsal head. Severe cases unresponsive to everything may require excising the sesamoid.

Sesmoiditis : Did you have an x-ray to make sure it's not a fracture ? If no fracture have a dancers pad added to orthotic which will take weight off area. Which sesmoid is it, tibial or fibular?
