A number of things,: Most common would be losing protein from the blood and the antibody level would correlate with low serum albumin levels (intestinal or kidney disease). Severe malnutrition is another possibility. Medication side effect is another (eg prolonged prednisone treatment). Finally, primary immune deficiency with poor B-cell function is possible and would correlate with history of infection
