Get checked: Severe cramping can be a result of intestinal causes or uterus causes. If pain is this severe perhaps an exam by a doctor is indicated in person. This way proper testing and medication can be prescribed. Please see your doctor as soon as possible or go to the emergency room if your pain is this severe.

Get urgent attention: Its not clear from your question how long you have been suffering, or whether your symptoms are related to your period, to a recent activity or strain, or to any associated symptoms like fever, bleeding, vomiting or urine changes.
