PDD spectrum?: Has he been evaluated by a developmental pediatrician? He might be in the PDD spectrum and he may benefit from therapies like ABA or OT. IT would be a good idea to get evaluated by a developmental pediatrician.

Behavioral illness: He may have some underlying behavioral illness or even a form of autism. Have him see a developmental specialist to assure he gets the proper medical care if he need it. Best wishes.
