See below: Clinical features and lab testing are not specific, a high index of suspicion must be maintained for the diagnosis. Cultures can be done growing the bacteria. Serologic test: labs and include the microscopic agglutination test (mat), macroscopic agglutination test, indirect hemagglutination, and elisa. Mat is considered the gold standard. Molecular techniques (pcr) are being developed.

Leptospirosis: The most common way to diagnose leptospirosis is through serological tests either the Microscopic Agglutination Test (MAT) which detects serovar-specific antibodies, or a solid-phase assay for the detection of Immunoglobulin M (IgM) antibodies.” REF: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3939550/#:~:text=The%20most%20common%20way%20to,Immunoglobulin%20M%20(IgM)%20antibodies.
