Quadriceps strain: As a sports radiologist, I have seen muscle tears missed on MRI because the area of injury wasn't included in the images. Seek a second opinion for your MRI to be sure. Persistent pain after a year suggests that you are either re-injuring the same region so that it doesn't heal properly, or the initial injury was more severe then your doctor believes. Has your PT tried therapeutic ultrasound?
