Excisional biopsy: What did the imaging study (CT or MRI) of the neck show? was there any suspicious finding on the head and neck exam that would suggest a diagnosis and possible primary? whats the differential diagnosis on your father? basically between reactive to infection or neoplastic? you would also ask about potential risk for nerve damage and impairment based on extent of dissection needed.

Risks, complications: necessity of operation, and anticipated information that would be gained from the biopsy. If it is posterior triangle lymph node, it could be close to the accessory nerve. Good luck.
