No spread: If there is any question by your physician then a bx should be performed. He may feel it is large, firm, and has the possibility of being malignant. When such a process in a node is present and especially when no head and neck primary can be found then one must rule out lymphoma and this is only by excisional bx. This type of bx does not cause spread and can only help if proper dx made.
