Could be: Cold sores, infection. Truly its really hard to say unless we see the red spots. When in doubt have your doctor check it out. Good luck

Wait and watch: The red spots on your child lips could be due to allergy , mild trauma,beginning of viral infection or prominent blood vessel .Look for such spots if they appear on other parts of body .Since they are not bothering watch and wait . If any change in spots -size ,color ,any discharge or bleeding ,then consult your doctor
