A 35-year-old female asked:

are isoechoic and hypoecoic nodules and cysts cancerous?

Are you referring: to breast ultrasound? Simple cysts are benign, cysts with solid components are more suspicious. Isoechoic and hypoechoic masses can be benign or malignant. The probability of malignancy depends on other ultrasound features. Some need biopsy, some can be followed or ignored.

Unlikely: Most ovarian cysts resolve on their own or with treatment. Occasionally, they leak the cyst fluid within (ruptured cyst) or twist (ovarian torsion). When that happens there is usually some degree of pain.

Various cell types: Endometrial cancer is characterized by stage and cell type. Clear cell is a description of the type of cells and simply behave in a more aggressive growth pattern than others.

The clear cell doesn't make it more aggressive, but it is a description of the cell type. Endometrial or ovarian cancer when it is poorly differentiated (grade 3) has a more aggressive behavior, made more so when stage is include (extent of disease).

The difference between appendicitis and a ruptured cyst?

Huge but simlar pain: There is a huge difference between appendicitis and a ruptured cyst. However the initial symptoms may be nearly identical making the differential diagnosis confusing. Appendicitis requires close observation, antibiotics, and possible surgery for impending rupture. However rupture ovarian cysts rarely require intervention other than pain medication unless there is sever associated bleeding.
