This refers to the: finding on an X-ray of the abdomen, which shows two large air filled spaces, the double bubble sign. The air is trapped in the stomach & 1st part of the duodenum(upper part of intestine),which are separated by the pyloric sphincter, creating the appearance of two bubbles visible on x-ray.There is no air seen in the distal duodenum because the atresia completely closes it off.
