Tough to treat: But consider use of Lyrica, at high dose, Cymbalta, Savella, (milnacipran) nortryptyline, Stelazine, Gralise, Horizant, but also a couple of different opioids, such as Nucynta or Butrans patch. Some useful exercise programs may make a difference, and many physical therapists have this knowledge. Acupuncture and biofeedback may work.
