May be neither: Smoking of itself does not cause total body pain however there is a condition called Polymyalgia Rheumatica that can accompany a cancer situation Smoking increases cancer risk. Arthritis refers to joint pain not total body pain. Infection and certain kinds of allergic reactions can cause total body pain as can certain medications in patients sensitive to them.
