What is the question: i understand you had upper endoscopy that showed gastritis and apparently you have had surgery for reflux in the past with evidence of nissen fundolpication and you had a motility test showung disturbance in the esophagus
the biggest question is what is your problem you were so occupied of reporting the result you forgut to tell us what is the question

Irritation: Results like this are fairly common and not specifically indicative of any particular disease. Often the Gastroenterologist will try an acid blocker to relieve symptoms. One of the main reasons they do the biopsies is to rule out cancer, and nothing you noted above sounds like any kind of cancer. Discuss the findings, and their relation to your symptoms, with the GI Dr. who performed the endo
