Unlikely but possibl: As a general rule, less invasive and potentially toxic therapy would be tried first, but without examining you and assessing the full clinical picture it is difficult to know. Listen carefully to the dermatologist and ask questions about side-effects of accutane. Good luck and hope your acne clears soon.

Yes: Accutane is not without risk. You have to be ABSOLUTELY sure that you do not become pregnant. Also, frequent nosebleeds and gastrointestinal upset are likely. Be sure that you talk in depth with your doctor about the risks, so you know what you are getting in to. Hope this helps!
