Follow up: With your health history and habits I would complete the treatment and go back to the dentist for any follow up and additional treatment needed. You should not worry too much as long as you follow your dentist recommendations. Take it easy.

You should: Pain in your chest is a serious medical condition. It is always a red flag and appointment with your PCP is warranted, especially if you smoke and drink. See your doctor for further investigation, tests and treatment. A tooth abscess or gum abscess won't go away without dental treatment. See a dentist for evaluation and x-ray. And yes, quit smoking and drinking.
