Are you asking: What could possibly be causing the symptoms? I think it makes more sense to have a physical exam and treatment, given this is not a scenario where context is provided. Mastitis, infection of the breast is concern #1, and breast cancer is concern #2. Having the two coexisting seems unlikely, but possible.
