A few possibilities: Not sure if the voltaren (diclofenac) is causing renal problems since it is an NSAID pain medication. Your issue may be more musculoskeletal, meaning not kidney related, but the best person to evaluate would be an orthopedist, a sports medicine orthopedic specialist, or possibly a physiatrist(rehab medicine). If you are an athlete or very sports minded this is where I'd get checked out first to get answers

Typical kidney infec: You have a classical kidney infection. You need 21 days of appropriate antibiotic treatment -- preferably Ampicillin, 500 mg 4 times per day. It will fade away as treatment proceeds.
