Autoimmune disorder : your mother has signs and symptoms of some type of autoimmune disorder such as rheumatoid arthritis or lupus. Chronic infections such as TB or endocarditis also a possibility. She needs further work up and referred to a specialist, possibly rheumatologist

If you both are not happy with the information supplied, time to see someone else, a pulmonologist or a rheumatologist are both possible choices of specialists who may be able to help. Good luck.
