What is the treatment of chondromalacia patellae grade 3 to early 4? i read about arthroscopy and platelet-rich plasma and stem cell transplantaion.

Tough problem: First try all of the conservative options: bracing, therapy, steroid injections, viscosupplemention injections (synvisc, Supartz, (hyaluronic acid) etc). If this fails then sometimes an arthroscopy may help. The science is still out on PRP.
