Abnormal: It means there are changes of atherosclerosis in the arch of the aorta (the arch is the 160° bend the aorta makes after arising from the heart going north towards your brain, as it turns around and heads south towards your feet). On chest x-ray, which is a 2 dimensional view of a 3 dimensional structure, the arch is referred to as a "knob".
