Distinct layers: Pachymeningitis refers to inflammation (itis) of the covering of the brain known as the dura mater or pachymenix. Leptomeningitis refers to inflammation of the leptomeninges, which are the pia and arachnoid membranes that sit between the dura and the brain. Sometimes an MRI will show enhancement preferentially in one of these compartments over the other but typically they go together.
