Total removal: If surgery is for a malignant lesion, total hysterectomy bilateral salpingoophorectomy is needed. A prophylactic or therapeutic omentectomy should be performed since omentum is one of the first sites of metastasis due to TGF-B. Any visceral nodules should be removed if present with no residual lesion > 1cm. Chemo would then be necessary of mets present.
