See a urologist: Or dermatologist. No harm using miconazole but this is useful to clear yeast or fungus, not cuts. Why do you have cuts???? Sounds like you will better off after circumcision. This is a quick, easy procedure.

? Prepucioplasty: Foreskin keeps splitting because you have a degree of phimosis (it's too tight to retract easily) Now unlikely to respond to steroid cream due to scarring. Next most effective treatment would be prepucioplasty. Vertical incision is made thu constricting area on upper surface & sutured transversely. Skin will now be loose & retract easily & avoid further splits once it has healed.
