Where in ankle?: Posterior impingement pain from a prominent bone (os trigonum) or tendon (fhl) impingement can be worsened with activities that involve repetitive flexing like driving. Having the ankle in a dependent position for prolonged periods can aggrevate a variety of conditions. If bad enough, see an orthopaedic ankle specialist.

Position: Depending on the location of the ankle pain it could just be the postion of your foot as you drive, especially if you are driving long distances.
