Burn and infection: This requires immediate therapy with antibiotics, probably IV. Hand infections can be destructive. Get treatment ASAP.

Go to ER: I would seek immediate medical attention. Firstly the burn is in the extremity and depending on percent of area of skin involved it could be significant. Secondly it sounds like it has now become infected and pt has a fever, so there is concern for sepsis. Take to ED!
