What are some reasons a positive ana can become negative and then positive again.

Par for the course: The immune system is not a stagnant one and in fact likely the most active system in the body. Thus the level of activities vary constantly. However, it would be unlikely for a high ANA titer (1:128) or above to become negative altogether at any time without medication.
