Your rights: You have a right to see and have things explained. Ask your doctor. This does not appear to be a very complex entry - each office uses slight variations. But I'd mean that there are no documents on record related to social concerns. If it were "Criminal doc: none" I'd think it meant there's no criminal record. Ask - it may be a very useful way to begin to talk.
