Possibly: It may be related to you taking the medication around food consumption. While in general food has limited effects on tramadol absorption, it is known that with high fat meals you can see up 25% decreased peak blood level for tramadol(Ultram) ER. At 5AM it's unlikely that you have eaten but I suspect that's not the case at 10AM.
