Numerous conditions: Cracking is most often benign - if there is grinding with the cracking it could indicate arthritis behind the knee cap. Numerous conditions could cause pain and swelling and giving way, such as meniscus injury, ligament injury, patellofemoral arthritis, patellar misalignment, etc, so unfortunately you'll need to see a doctor to get a better diagnosis.
