Narcolepsy: Narcolepsy is a disorder of sleep regulation characterized by rapid entry into REM sleep, muscle weakness, daytime sleepiness, 'micronaps', hallucinations, etc. It can be very debilitating and dangerous if untreated (accidents, poor work performance, low quality of life, etc.) A sleep study would be an appropriate test to start an evaluation for this neurologic condition. Get back in rhythm: Sounds like you need to get to bed on a routine basis at the same time every night. Turn down the lights. if you don't fall asleep in an hour or so get up and walk around. Try not to think about things while you are trying to sleep. No coffee or tea before bedtime and don't be drinking alcohol as it will keep you up.
