Look at skin: If he's only been treated for pinworms once it might indicate a treatment failure. Local irritation from retained waste or tissue paper on the skin is also a possibility. Has the doctor examined the actually area or have treatments been prescribe based on symptoms? Look at the skin where it itches - is it inflamed/red? Is there a rash? If it looks totally normal then I would still consider worms.

Treat again: if he had intestinal worm, he may need testing and treatment again. Constipation may also be a contributing factor to anal area discomfort
