Couple issues: Could be some breakthrough bleeding or an infection causing pain or bleeding, but because the pill is not 100% effective you need to check a pregnancy test. You are also at the age where disorders like fibroids, endometriosis and adenomyosis may be symptomatic even on the pill. Its probably time to see a GYN.
