Not in the U.S.: No commercially available formulation of Levaquin (levofloxacin) I am aware of in the U.S. would ever be labeled to be good after that length of time. Whether chemically it has deteriorated likely depends on many factors but no doctor and no pharmacist would ever risk themselves by telling you it's OK to use it. There must be an expiration date on the bottle or box. Did you look?
