All of the above: You can develop structural abnormalities later in life: called acquired heart disease. This can be degenerative, or due to infection, or deficient circulation, or tumor, or injury - to name a few. Or you can be born with structural abnormalities (congenital disease) which may or may not be due to genetic abnormalities (inherited or familial).
