Many things.: Could be many things. I see that you have already had some testing done This will be very difficult to solve on line. You need a good clinician who can listen to your concerns.

Arthritis?: Perhaps psoriatic arthritis. It causes joint swelling and sometimes a rash, and it can cause flares. Visit with a rheumatologist or internist if you can.
