Yes it could: There are several changes on ECG that might indicate right sided chamber enlargement or strain. This may be acutely or chronically. What you may be asking is could these right sided abnormalities occur without ECG changes and yes that might happen too. An echocardiogram would help clarify, if there is a significant suspicion of right sided abnormalities.
