Varies, be seen: You are right, there are a number of possibilities including the lung and the spine. Women are also notorious for having cardiac ischemia presenting in unusual ways. When pain runs into the left shoulder like you are describing, it is concerning for a cardiac event. I would encourage you to be seen just to make sure it is not something potentially life threatening.
