Blow Out!: Without having a bit more information it's difficult to tell you the meaning of this one. It may refer to inserting a needle directly into a nerve when performing a block. Causing the nerve to be ruptured(blow out) during injection of anesthetic! The damage could be irreversible. Local anesthetics are injected around nerves not inside them. Using technology, such as ultrasound, helps avoid this.
