Exam is important: Could be a local myofascial tenderpoint. Since the imaging and other examination is normal would suggest you discuss with your physician about injecting the tender location. We see this and call this a tenderpoint. This can be where the muscles, and soft tissue come together.

Possibly msk: This may represent musculoskeletal pain or persistent abnormal signal from nerves that may have been divided during the surgery. There are medications that can be useful for primary nerve based pain. If musculoskeletal, conservative therapies such as PT or massage therapy can help. Recommend seeing DR to discuss.
