Shingles: It could be shingles, but without seeing the rash, would need to consider other diagnoses as well such as impetigo or celluitis. Follow up with your doctor.

Painful could be key: Is it just on one side of his back? Are there any blister-like areas? Does it follow a line along the path of a rib? Was there any pain or discomfort in the area before the breakout? If yes to these questions, it could be shingles. It is impossible to tell without actually looking at it. See your doctor for an accurate diagnosis.
